Patient Information

Minor Patients

All patients under the age of eighteen must be accompanied by a parent or guardian at the
consultation visit.

Medical Conditions

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our practice. X-rays from your dentist or physician must not be older then 3 months.

Appointments

  1. Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. At that time questions will be answered, the necessary procedure is determined and discussed, a short health history is taken and we will schedule you for a surgical procedure if you so desire.
  2. The surgery may be done at our practice or in theatre at the hospital. After surgery you will usually have one or two post-operative appointments to check your recovery. Normally no surgery is performed on day of consultation.
  3. Please assist us by providing the following supporting information at the time of your consultation: Your doctor’s referral letter and any x-rays if applicable.
  4. A list of medications you are presently taking.
  5. If you are a member of a medical aid scheme, bring the current medical aid card as proof.
  6. Main member of medical scheme valid identification document (ID) AND your (the patient) valid identification document (ID).
